Social Worker
This short term, part-time (20 hours per week) position supports activities of the Asian Pacific Islander Dementia Care Network and is responsible for information & assistance and some care management for families affected by dementia. This position will: a) assess the social, emotional, and other needs of families affected by dementia and formulate care plans to address them, b) provide counseling, education, skills training, and coaching for families, and c) identify ommunity resources and makes referrals to appropriate community resources. The successful applicant will have an ability to maintain a professional demeanor in a fast paced environment, be proficient with word processing, spreadsheet, communication software, applications, and designated equipment, and be available for occasional evening meetings and other event support that requires a flexible schedule.
