Bridge to Healthy Families
Bridge to Healthy Families (BHF) is a program that seeks to improve access to comprehensive support services for family caregivers and their care receivers.
The program offers caregivers:
- Care assessments and home safety check
- Consultation with understanding professionals
- Benefit checkup and application assistance
- Long-term care planning to help you prepare for the future
- Peer support groups and Educational programs
- Information and referral for respite care, legal/financial planning, transportation, in-home care, home modification, medical alert/security devices, care transition options, and more

Eligibility:
- Caregiving families for older adults residing in Sacramento County
