Aging & Disabilities Resource Connection

Aging and Disability Resource Connections (ADRCs) serve as single points of entry into the long-term services and supports (LTSS) system for older adults, people with disabilities, caregivers, veterans and families. Some states refer to ADRCs as “access points” or “no wrong door” systems.

Mission

To improve access to independent living for older adults and people with disabilities through person-centered services that work together for the benefit of the individual.

ACC Senior Services is a core partner of Emerging ADRC of Sacramento County. Other core partners are Agency on Aging Area 4 (AAA4) and Resources for Independent Living (RIL).

Services Offered

ADRC provide 4 core services:

  • Enhanced Information and Referral
  • Person-Centered Options Counseling
  • Short-Term Service Coordination
  • Person-Centered Transition Support

Who We Serve:

ADRC services are available to older adults, people with disabilities, and caregiving families regardless of age or income.

There is no cost to receive services. Voluntary contribution is gladly accepted to support the program.

Monday-Friday 8:30a-5:00p

Inquiries can be made via phone, email, and online form anytime, and will be responded to by a staff within 2 business days.
